I, Holly Elizabeth Vance respectfully petition this Court to change my name, pursuant to Indiana Code §34-28-2-1. In support of this petition, I state:

  1. My current legal name is Holly Elizabeth Vance.
  2. My date of Birth is 08/23/1987.

  2. Previously, my legal name was Holly Elizabeth Starley.
  3. I was born in Gainesville, Georgia and am a United States Citizen.
  4. I do not hold a valid US passport.
  5. I have never been convicted of a felony.
  6. I am not a registered sex or violent offender who is barred by I.C. § 11-8-8-16(a) from changing my name.
  7. My petition is made in good faith and not for fraudulent purposes. I want to change may name to Holly Elizabeth Robertson so that my name matches my domestic relationship.

THEREFORE, I respectfully request that this Court grant my Petition for Change of Name, and for all other just and proper relief.
