Comes now the Petitioner, Imer Fujzuli, being duly sworn and upon his oath, declare:

1. Your Affiant is the Petitioner for a Dissolution of Marriage.
2. The Respondent is Andrea J. Holt
3. The Parties were married on August 25, 2014 and separated on or about December 25, 2014; Affiant has not seen nor heard from the Respondent since that time.
4. The last known address that Affiant had for the Respondent is 6267 N. Clinton Road, Apt 2, Terre Haute, Indiana which was the address of Respondent when the Parties met.
5. Affiant does not know any of Respondent family, has searched Court records and social media (specifically, Facebook) and has no way of contacting the Respondent.
6. Your Affiant has learned that a warrant for the arrest of Respondent is outstanding in 61C01-1702-CM-000042.
